You can submit an online query regarding a fit note, recent test results, recent referrals or repeat prescription queries. You will need to create your Patchs account first.
If we receive an online admin request that we feel is not appropriate you will receive a response directing you to telephone the surgery.

Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours. The system will not be available over the weekends (Saturdays and Sundays), bank holidays and they will not be read Monday-Friday between the hours of 1800 – 0900. Please contact 111 during out of hours or 999 if this is an emergency.
