Confidentiality Statement

The doctors, staff and other NHS employees have a legal duty to maintain the highest level of confidentially about patient information. We will not disclose your information to third parties, including relatives, friends and carers, without your permission, unless there are exceptional circumstances, such as when the health and safety of others is at risk.

 

Protection and Use of Your Information

We need to ask for personal information to help ensure that you receive the best possible care and treatment. This information is kept on our fully computerised system, together with details of your ongoing care, so that your doctor or nurse has access to accurate and up to date information.

To enable us to maintain accurate records we ask that you tell us of any change in your particulars: name, address, telephone number etc.

There are times when we have to pass on information about you to other people such as hospitals, Health Authority or the Social Services. This is always done confidentially and any identifying details are removed when possible.

We have a legal duty to maintain the highest level of confidentiality about our patient information. All data which can identify a person held in any format (visual, verbal, computerized) is also safeguarded by the Data Protection Act 1998.

Freedom of Information Act (FOI) - Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available. To view a copy of the scheme please click here. 

 

Patients Charter
All members of the Danebridge Medical Centre Practice Team are dedicated to a quality service, in order to achieve health services, which meet the patients needs. To view a copy of our Patients Charter please click here  

 

Zero Tolerance

We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises.

 

Chaperone Policy

If you would like to have a chaperone present during an examination or consultation, please arrange this with the doctor.

 

Half Day Closing
Most practices in the South Cheshire area will be closing for one half-day each month to provide an opportunity for Doctors, Practice Nurses and support staff to do valuable training and development work.
 

On these dates all surgeries close at 1pm.  However, Danebridge and Kingsmead will re-open at 5pm for emergencies only.  During the closed period, emergencies are dealt with by the Out of Hours Service by telephoning: 01270 273666

Wednesday, 11th June
Thursday, 3rd July
Tuesday, 9th September
Wednesday, 8th October
Thursday, 6th November
